Scheduling an appointment with Young Nephrology is simple. Just give us a call. 

If you’ve been referred by your primary care physician or another healthcare provider, please ask them to send your referral and any relevant medical records to our office. 

For patients contacting us directly due to concerns about kidney health, we’ll help you determine the next steps and answer questions before your visit. 

Not sure whether you need to see a nephrologist? We’re happy to help you understand the referral process and decide whether an appointment is appropriate.

Preparing for your Office Visit

Your first appointment is designed to help you understand your kidney health. 

Dr. Young will review your medical history, medications, lab results, and any imaging studies related to your kidney function. He will take time to explain what those results mean, answer your questions in clear language, and discuss next steps together before recommending a personalized treatment plan. 

Your Appointment

To help us make the most of your appointment, please bring the following items: 

  1. Your photo ID and insurance card(s)

  2. A current list of all prescription and over-the-counter medications 

  3. A list of questions or concerns you’d like to discuss 

Bringing a current medication list helps Dr. Young better understand your kidney health and allows more time during your visit to focus on your questions and care plan. 

Patient Forms

You must complete your new patient forms 1-2 weeks prior to your first visit.

You can mail or fax them to the office, or bring them to the front desk of the office in Lake Saint Louis.
